Please feel free to use this form to enquire about whether we are the right service for you, or if you have any questions before booking an appointment with us.

At your initial consultation / assessment with Mindful Nutrition Practice, you will also have the opportunity to discuss your individual needs, how you would like to work together and what you would like to work on.

When booking an appointment date and time, you will be asked to complete a secure online registration and consent form to confirm the appointment. After this has been done, you will be sent the relevant paperwork before your appointment via our online platform, including a pre-assessment questionnaire.

The initial consultation will be an assessment, and we will discuss how we can work together and what you would like to get out of treatment with us. We are here to support you in achieving your goals and working on food recovery and symptom management. You will have the opportunity to ask any questions you have in a safe and non-judgemental space.

When booking an appointment below, please ensure you complete all of the information required, which includes filling out your information, a consent form and payment details. Make sure you scroll down and press the “next” button after completing each page.

Your appointment will not be confirmed until you enter your payment details to secure your booking. You will only be charged after your appointment or in the event of late cancellation / change of appointment (within 48 hours of the appointment).

You will receive an email confirmation once the appointment has been booked. If you have not received this within 10 minutes, then please try again and ensure you have completed the booking process as above, or feel free to get in touch.
